We are launching a new and quicker way to book appointments and send requests to the practice this summer. For most people, it will be the fastest way to get an appointment and ensure that you are seen by the right person at the right time.
By providing online triage, it will help to make sure we see people with the greatest need first, and free up our GPs to spend more time caring for patients. It’s designed mainly for new health problems, though you will also be able to use it for simple admin requests like a sick/fit note.
Here's some further information on how Smart Triage will work.
1. What is Smart Triage?
Smart Triage is an NHS-approved online triage and appointment booking system. It asks you a short series of questions and then directs you to the most appropriate care. The most likely outcome is to be offered an appointment.
2. Why are we introducing it?
We know it can be difficult to book appointments. Smart Triage helps us respond faster, see the most urgent cases first, and lets GPs spend more time on care and less on admin.
3. When will we launch it?
Smart Triage will launch on Monday 1st June and can be accessed from our practice website.
4. Who can use it?
The system is for patients registered at this practice. Nursing services (eg blood tests, vaccinations, cervical screening), long-term condition reviews, and some specialist clinics will still be booked through reception. Those without online access (or those that need a translator or home visit) should still phone the practice, as normal.
5. How do I use it?
The link will be added to our home page on 1st June.
Complete the short online form (about 3½ minutes).
If you cannot go online, call or visit reception and a team member will complete the form with you.

6. What happens after I fill in the form?
Based on your answers, Smart Triage will:
- Offer you an appointment (telephone or face-to-face) if one is appropriate and available, or
- Send your request to the practice team for review.

7. Do I need to share personal details in the form?
Yes, but only relevant questions that help direct you to the right clinician. Your answers are saved securely and remain confidential.
8. Can I book my preferred appointment online?
Yes, where suitable and available, Smart Triage offers one or more appointments. These may be pre-bookable appointments or same-day appointments, depending on the nature of the problem.
9. What if I need a sick / fit note?
You can request a sick / fit note through Smart Triage.
10. I need an interpreter, home visit, or special assistance, what should I do?
Please phone the practice reception, so we can arrange an interpreter, home visit, or the assistance you need.
11. What must I do before launch?
Please check that we have your correct email address on your patient record. You will need this to access Smart Triage. You can update your details via the NHS app.
